Premise Health is seeking a highly skilled Physical Therapist to join our team in Northbrook, IL.
Job Summary:We are looking for a Physical Therapist to work in collaboration with other Health Center staff members to promote health and wellness to onsite employees and their dependents. The ideal candidate will have a strong background in outpatient orthopedic physical therapy and experience working in an ambulatory care setting.
Key Responsibilities:- Develop and implement treatment plans for patients with a variety of musculoskeletal and movement disorders.
- Conduct thorough assessments and evaluations to identify patient needs and develop effective treatment plans.
- Provide direct access to physical therapy services and work closely with other healthcare professionals to ensure seamless care.
- Participate in continuing competence activities, including peer review, presentations, and grand rounds.
- Develop and administer injury prevention and health promotion programs for onsite employees and other eligible populations.
- Current state licenses/certifications to practice physical therapy.
- Board certification (if applicable).
- Degree from an accredited physical therapy school.
- CPR certification (BLS for the Healthcare Provider/Professional Rescuer).
- At least three years' experience as a Physical Therapist in an outpatient physical therapy clinic or other ambulatory care clinic.
- Outpatient orthopedic experience is required.
- Knowledge of Internet software and Word Processing software.
- Knowledge and experience with Electronic Medical Records.
- Knowledge of workplace health and safety concepts and OSHA regulations in occupational health settings.
