VNS Health is seeking a highly skilled Nurse Practitioner to join our Hospice Care team in Queens, New York.

Job Summary

As a Nurse Practitioner on our Hospice Care team, you will provide advanced nursing clinical care to patients in their homes, delivering compassionate and high-quality care that meets the unique needs of each individual.

Responsibilities
  • Provide advanced nursing care and treatment to hospice patients, including assessment, diagnosis, prescribing medication, and ordering treatments and DME.
  • Perform in-person pre-admission visits and complete hospice admissions, as well as re-certification visits for continued service in the Hospice Program.
  • Collaborate with patients, families, primary care physicians, and other team members to provide assessment and care planning.
  • Consult with primary care physicians and specialists as needed, and provide medical and nursing expertise to patient care staff on issues and challenges involving evaluation of terminal decline, pain and symptom management, patient/caregiver education, and proactive care planning.
  • Participate in on-call NP coverage rotation and provide care in accordance with VNS Health policies, practices, procedures, and Standards of Nursing Practice.
Requirements
  • Licensed and currently registered to practice as a Nurse Practitioner in New York State.
  • DEA license required.
  • Valid driver's license may be required, as determined by operational/regional needs.
  • Master's Degree in Nursing required.
  • Minimum of three years clinical nursing experience required.
  • Experience as a Nurse Practitioner preferred.
