We are seeking a skilled RN Case Manager to support our patients with complex chronic illnesses. As a DaVita Integrated Kidney Care team member, you will play a critical role in delivering high-quality patient care and improving outcomes.

Key Responsibilities:

  • Assess patients' medical, social, and emotional needs
  • Develop and implement individualized care plans
  • Coordinate care transitions and manage ongoing needs
  • Identify and address social determinants of health
  • Maintain effective relationships with patients, families, and healthcare teams

Requirements:

  • RN license with 2+ years of experience
  • Current CPR/BLS certification
  • Ability to travel within assigned territory
  • Intermediate computer skills and proficiency in MS Office

Salary Range:$62,000 - $90,000 per year
