Key Responsibilities- Develop and implement medication therapy plans for patients, taking into account their medical history, current health status, and treatment goals.
- Monitor patient care and adjust medication therapy plans as needed to ensure optimal outcomes.
- Collaborate with healthcare teams to provide comprehensive patient care and ensure seamless communication.
- Provide education and training to patients, families, and healthcare professionals on medication use and management.
- Participate in quality improvement initiatives to enhance patient care and medication use processes.
- PharmD or MS degree from an accredited institution.
- Current licensure to dispense controlled substances in the State of Utah.
- Ability to work effectively in a fast-paced environment and prioritize tasks to meet deadlines.
- Excellent communication and interpersonal skills, with the ability to work collaboratively with diverse teams.
- Strong analytical and problem-solving skills, with the ability to interpret data and make informed decisions.
- PGY2 Critical Care Pharmacy Residency.
- Experience working in a hospital or healthcare setting.
- Knowledge of medication use policies and procedures.
This position involves light work that may exert up to 20 pounds and may consistently require light work involving lifting, carrying, pushing, pulling or otherwise moving objects. This position involves standing for extended periods of time and is not exposed to adverse environmental conditions.
