Job Summary:

Avera@Home Yankton is seeking a highly skilled Registered Nurse to join our team as a Case Manager. In this role, you will be responsible for patient care planning, coordination, and education within a multidisciplinary team.

Key Responsibilities:
  • Develop patient care plans in collaboration with the physician and interdisciplinary team
  • Coordinate patient care services and educate the interdisciplinary teams
  • Provide patient-centered care efficiently and effectively to achieve optimal outcomes
Requirements:
  • Graduate from a CCNE or NLN accredited nursing program (strongly preferred)
  • Bachelor's degree in nursing (preferred)
  • Minimum 2 years related experience and/or training; or equivalent combination of education and experience (preferred)
  • License as a Registered Nurse (RN) in South Dakota and/or meeting the requirements of Multi-state licensure (required)
Benefits:
  • Competitive hourly rate: $29.50 - $44.25 per hour
  • Sign-on bonus: $20,000 with a 3-year commitment
  • Mileage reimbursement
  • Flexible work schedule: 8:00am-4:30pm with wknd/hol/call involvement; 72hrs/2wks
