Nurse Care Coordinator

The Agency LPN at Nia Association plays a vital role in providing exceptional nursing care to individuals with intellectual and developmental disabilities.
Key Responsibilities:- Develop, implement, and evaluate individualized care plans in collaboration with the healthcare team and families.
- Administer medications, treatments, and other prescribed medical interventions in accordance with policies and regulations.
- Monitor and manage acute and chronic medical conditions, recognizing changes in health status and responding promptly.
- Collaborate with residential staff, therapists, physicians, and families to ensure holistic care and support for service recipients.
- Maintain accurate medical records, including progress notes, medication administration logs, and incident reports.
- Educate staff and service recipients on health-related matters such as nutrition, hygiene, and self-care techniques.
- Participate in regular case reviews and contribute to the development of individualized service plans (ISPs).
- Ensure compliance with healthcare regulations and best practices, including HIPAA and OSHA standards.
- Provide emergency first aid and triage care as needed.
- Conduct and/or coordinate Medication Administration Classes and assist with ongoing training and support of direct care staff regarding health and medical care for service recipients.
- Maintain a monthly calendar of medical appointments for the assigned individual(s) in the agency.
- Review Physician Order Encounter Forms to ensure Prescription Protocols have been followed.
- Current RN or LPN licensure in the state of Tennessee.
- Minimum of 2 years of nursing experience, preferably with individuals with intellectual and developmental disabilities.
- CPR and First Aid Certification.
- Knowledge of healthcare laws, guidelines, and procedures relevant to nursing in a residential care setting.
- Strong clinical, organizational, and communication skills.
- Ability to work both independently and as part of a multidisciplinary team.
