The Agency LPN at Nia Association plays a vital role in providing exceptional nursing care to individuals with intellectual and developmental disabilities.

Key Responsibilities:
  • Develop, implement, and evaluate individualized care plans in collaboration with the healthcare team and families.
  • Administer medications, treatments, and other prescribed medical interventions in accordance with policies and regulations.
  • Monitor and manage acute and chronic medical conditions, recognizing changes in health status and responding promptly.
  • Collaborate with residential staff, therapists, physicians, and families to ensure holistic care and support for service recipients.
  • Maintain accurate medical records, including progress notes, medication administration logs, and incident reports.
  • Educate staff and service recipients on health-related matters such as nutrition, hygiene, and self-care techniques.
  • Participate in regular case reviews and contribute to the development of individualized service plans (ISPs).
  • Ensure compliance with healthcare regulations and best practices, including HIPAA and OSHA standards.
  • Provide emergency first aid and triage care as needed.
Additional Responsibilities:
  • Conduct and/or coordinate Medication Administration Classes and assist with ongoing training and support of direct care staff regarding health and medical care for service recipients.
  • Maintain a monthly calendar of medical appointments for the assigned individual(s) in the agency.
  • Review Physician Order Encounter Forms to ensure Prescription Protocols have been followed.
Qualifications:
  • Current RN or LPN licensure in the state of Tennessee.
  • Minimum of 2 years of nursing experience, preferably with individuals with intellectual and developmental disabilities.
  • CPR and First Aid Certification.
  • Knowledge of healthcare laws, guidelines, and procedures relevant to nursing in a residential care setting.
  • Strong clinical, organizational, and communication skills.
  • Ability to work both independently and as part of a multidisciplinary team.
