Job Title: Home Health Occupational Therapist

Supplemental Health Care is seeking a skilled Occupational Therapist to join our team in Lincoln City, Oregon. As a Home Health Occupational Therapist, you will work with patients in their homes, providing personalized therapy and support to help them achieve their goals.

Job Summary:

  • Provide occupational therapy services to patients in their homes
  • Develop and implement individualized treatment plans to meet patient needs
  • Collaborate with healthcare team members to ensure comprehensive care
  • Conduct assessments and evaluations to identify patient needs and goals
  • Implement and document treatment plans, including progress notes and outcome measures

Requirements:

  • Graduate of an accredited Occupational Therapist program
  • Licensed as an Occupational Therapist in Oregon
  • CPR Certified or ability to obtain
  • Previous experience with Computer Charting
  • 6 months of previous Home Health Care experience
  • Valid U.S. Driver's License and reliable transportation
