Position Summary
The Clinical Care Leader (CCL) plays a crucial role in delivering personalized nursing care within the hospice environment. This position requires collaboration with interdisciplinary teams to design, execute, and assess care plans. Responsibilities include documenting patient histories, symptoms, medications, and care provided in accordance with established standards and regulations. The CCL leverages technology to improve patient care quality, effectively delegates tasks, supervises unlicensed personnel, and maintains overall nursing accountability for assigned patients. Leadership roles may also be assumed in departmental and organizational committees.
The CCL must demonstrate comprehensive nursing knowledge and practice within the full scope of nursing expertise, adhering to age-specific standards relevant to the care area. Strong communication skills are essential for effective collaboration, negotiation, and conflict resolution with peers, healthcare professionals, patients, and their families. Critical thinking and clinical reasoning abilities are necessary for data analysis, care coordination, patient assessment, education, triage, and other nursing interventions.
The CCL operates within the frameworks and standards set forth in the Nurse Practice Act and Administrative Rules of the state of practice. The Sanford Professional Nursing Practice acknowledges the Scope and Standards of Practice as well as the Code of Ethics for Nurses, as established by the American Nurses Association, as foundational to nursing care delivery and professional behavior.
Qualifications
A Bachelor's degree in nursing is strongly preferred. Graduation from a nationally accredited nursing program is also preferred, including programs recognized by the Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).
A minimum of two years of experience in home health or hospice nursing is preferred.
Applicants must possess an unencumbered RN license from the State Board of Nursing in the practice area and/or hold multistate licensure if in a Nurse Licensure Compact (NLC) state. Candidates are expected to obtain and maintain required competencies and certifications specific to the department. Depending on the location, a valid driver's license may be necessary. Basic Life Support (BLS) certification is required, and certification as a Certified Hospice and Palliative Nurse (CHPN) is preferred.
