Role Overview:
Pathways Home Health and Hospice is in search of a skilled and empathetic Registered Nurse (RN) Case Manager with a focus on hospice care. This full-time position is designed for those who are committed to providing exceptional support to patients and their families during critical times.

Key Details:
  • Office Location: Sunnyvale
  • Patient Service Area: Mountain View - Atherton
  • Work Schedule: Full Time
  • Shift Hours: 8:30 AM - 5:00 PM, with rotating weekends and holidays
  • Sign-On Bonus: $9, Conditions Apply

Responsibilities:
  • Conduct comprehensive initial assessments of patients
  • Collaborate with a multidisciplinary team to coordinate care
  • Document all observations, findings, and interventions accurately
  • Evaluate the need for additional services from team members
  • Provide skilled nursing care and perform necessary procedures
  • Work closely with the Clinical Team Manager to ensure effective care
  • Maintain compliance with documentation and technology standards
  • Engage in quality improvement initiatives

Qualifications:
  • Valid California RN license
  • At least one year of nursing experience
  • Current CPR certification
  • Valid California driver's license and automobile insurance
