**Job Summary**

We are seeking a skilled and compassionate Registered Nurse to join our team as a Home Health RN. As a Home Health RN, you will be responsible for monitoring patients' overall health, developing individualized plans of care, and delivering quality healthcare services in a home setting.

**Key Responsibilities**

  • Monitor patients' health status and develop individualized plans of care in partnership with primary care physicians
  • Deliver quality healthcare services, including wound therapy, post-surgical care management, and ostomy care management
  • Provide patient education on disease management and customized care
  • Work independently to deliver care, with support from an internal operational team

**Requirements**

  • Minimum 1 year RN experience in acute, SNF, home health, or hospice setting
  • Able to administer IV medication and educate patients
  • Active CA Registered Nursing License
  • AA, BSN, MSN degree
  • Reliable transportation
  • Current CPR or BLS Card
