We are seeking a skilled Nurse Case Manager to join our team at Paradigm. As a Field Case Manager, you will be responsible for the medical case management of work-related injuries, which includes assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals.
This role is a home-based position with travel, up to 2 hours one-way, to doctors' offices, hospitals, and various other locations. You will work closely with insurance carriers, medical care providers, attorneys, employers, and employees to monitor the progress of the injured worker and report results back to the employer and insurance carriers.
Key Responsibilities:
- Conduct thorough assessments of injured workers to determine the extent of their injuries and develop effective treatment plans.
- Coordinate with medical care providers to ensure timely and appropriate medical care.
- Communicate with insurance carriers, attorneys, employers, and employees to ensure seamless case management.
- Monitor the progress of injured workers and report results back to the employer and insurance carriers.
Requirements:
- Current, unencumbered Registered Nurse (RN) license in GA.
- CCM, CRRN, CDMS, or COHN certification required.
- Experience in case management of workers comp cases preferred.
- Valid driver's license and good driving history.
