About the Role

We are currently seeking a highly skilled Occupational Therapist to join our interdisciplinary team. As a valued member of our care team, you will work collaboratively with patients, families, and healthcare professionals to develop and implement tailored rehabilitation plans. Your extensive expertise will enable patients to overcome physical, emotional, and cognitive challenges, fostering their independence and overall well-being.

Responsibilities:

  • Patient Assessment and Goal Setting: Conduct thorough assessments to identify patients' needs and establish realistic goals, ensuring seamless integration into our comprehensive care plan.
  • Treatment Planning and Implementation: Develop and implement evidence-based treatment plans that cater to patients' unique requirements, incorporating individualized interventions to promote recovery and independence.
  • Care Coordination and Discharge Planning: Collaborate with multidisciplinary teams to coordinate patient care, ensuring a smooth transition between healthcare settings and promoting timely discharges.
  • Staff Supervision and Training: Provide guidance and mentorship to Occupational Therapy Assistants and Home Health Aides, empowering them to deliver high-quality care and achieve outstanding patient outcomes.
  • Documentation and Communication: Accurately document patients' care observations, interventions, and evaluation, communicating effectively with healthcare teams to ensure seamless information exchange.

Qualifications:

  • Master's degree from an accredited Occupational Therapy Program.
  • A minimum of six months of occupational therapy experience, with preference given to candidates with prior home health experience.
