Job Title: Home Health Physical Therapy Assistant

We are seeking a skilled Physical Therapy Assistant to join our team at LHC Group, a leading provider of home health care services. As a Physical Therapy Assistant, you will play a vital role in helping patients recover from injuries or illnesses and regain their independence.

Job Summary:

The Home Health Physical Therapy Assistant will work under the supervision of a licensed Physical Therapist to provide high-quality patient care in the comfort of their own homes. Your primary responsibilities will include:

  • Providing exercise instruction and patient education to help patients achieve their rehabilitation goals
  • Assisting patients with active and passive exercises, muscle re-education, and gait training
  • Observing and recording patient progress, and reporting any changes to the supervising Physical Therapist
  • Coordinating care with other healthcare team members as needed
Requirements:

To be successful in this role, you will need:

  • A current license to practice as a Physical Therapy Assistant in the state of New Jersey
  • Excellent communication and interpersonal skills
  • A strong commitment to providing high-quality patient care
