Job Title: RN - Home Health

Are you a dedicated and compassionate Registered Nurse looking for a challenging and rewarding career in home health care? We are seeking a skilled and experienced RN to join our team at Bellin Health, a leading healthcare provider in Northeast Wisconsin.

Job Summary:

We are looking for a Registered Nurse to provide high-quality, patient-centered care to individuals in their homes. As a member of our home health team, you will conduct comprehensive physical assessments, develop and implement individualized care plans, and provide skilled nursing services to patients with a variety of health needs.

Responsibilities:
  • Conduct in-home visits to assess patients' physical and emotional needs
  • Develop and implement individualized care plans to meet patients' unique needs
  • Provide skilled nursing services, including IV medication administration, wound care, and Foley/ostomy care
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination
  • Document patient progress and outcomes in a timely and accurate manner
Requirements:
  • Wisconsin RN license required
  • Valid Wisconsin driver's license in good standing and liability car insurance required
  • Sign on Bonus available for qualified candidates
