Nutritional Care Associates collaborate closely with Registered Dietitians to improve the overall health and well-being of our residents. This role involves gathering health information and working alongside other healthcare professionals to implement tailored nutritional care plans.
Key Responsibilities include:
- · Assisting in the completion and updating of nutritional assessments for residents under the guidance of the Registered Dietitian.
- · Conducting resident evaluations to determine individual needs for nutrition services and identifying those at nutritional risk.
- · Collaborating with interdisciplinary team members to develop, execute, and assess care plans.
- · Monitoring resident weights to detect and report significant changes.
- · Performing regular meal observations, reviewing documentation, and engaging with staff and residents to ensure nutritional adequacy in food delivery.
Qualifications:
- · Required: Associate or bachelor's degree in Nutrition and Dietetics or equivalent, or actively pursuing a degree in a healthcare-related field, preferably in nutrition/dietetics, or possess a current Dietary Manager Certificate (CDM) with a Practitioner Certificate in Nutrition Care or equivalent.
- · Preferred: Registered Dietetic Technician.
- Certified Dietary Managers (CDMs) are encouraged to apply.
