Job Description

The Case Manager plays a crucial role in ensuring seamless healthcare services to our patients. This position requires a Bachelor of Science in Nursing degree and current California RN license. With three to five years of experience in an acute care hospital setting and previous one to two years as a Case Manager, you will possess the skill, knowledge, and ability essential to the successful performance of assigned duties. Your excellent written and verbal communication skills in English, proficiency in typing, and computer skills, including Microsoft Office, are also required.

The ideal candidate will be able to maintain a work pace appropriate to the workload, establish and maintain effective working relationships across the Health System, and provide exceptional customer service skills. American Heart Association BLS Certification and broad knowledge of Medicare, Medi-Cal, and insurance guidelines are preferred qualifications.
