Job Summary

We are seeking a skilled Registered Nurse (RN) to join our team as a Palliative Care Nurse. The successful candidate will work with patients, their families, and other healthcare professionals to provide and coordinate high-quality end-of-life care. This role requires strong clinical skills, excellent communication, and a compassionate approach to patient care.

Responsibilities
  • Conduct in-person and telephonic assessments to palliative care patients in collaboration with the interdisciplinary team.
  • Provide education to patients and families on disease management, self-care techniques, end-of-life care, medications, nutrition, and dietary needs.
  • Communicate findings to the Palliative NP and/or PCP as needed, but no less than bi-weekly.
  • Re-assess the effectiveness of care plans on a regular basis and modify as needed.
  • Collaborate with the Palliative Care Team to develop and modify care plans for each patient.
  • Participate in evening/weekend call as required, conducting on-call services in a clinically competent and responsive manner.
Requirements
  • Registered Nurse (RN) license to practice in the state.
  • 2 years of prior skilled Registered Nurse experience.
  • Demonstrated ability to provide quality patient care.
  • Nursing experience in palliative care, oncology, hospice, or geriatric care preferred.
