Job Description:
Cleveland Clinic Foundation seeks an accomplished Heart Failure Cardiologist to lead our Regional Cardiovascular Medicine program. As a member of our esteemed team, you will play a pivotal role in shaping the future of heart failure care across our regional hospitals.

About the Role:
Your responsibilities will include developing and implementing evidence-based systems of care for heart failure patients, collaborating closely with multidisciplinary teams to ensure seamless coordination. You will also be responsible for overseeing population health management initiatives, leveraging electronic medical record population health analytics to drive informed decision-making. Additionally, you will have the opportunity to participate in academic activities, including a faculty appointment at the Cleveland Clinic Lerner College of Medicine of Case Western Reserve University.

Qualifications:
We are looking for a dedicated professional with a strong background in cardiovascular medicine, holding BC/BE certification. Advanced subspecialty training in heart failure is highly valued. A collaborative approach, excellent communication skills, and a passion for delivering exceptional patient care are essential qualities for success in this role.
