Per Diem Nurse Case Manager

Mass General Brigham is seeking a skilled Per Diem Nurse Case Manager to join our team. As a Per Diem Nurse Case Manager, you will play a vital role in coordinating patient care and ensuring seamless transitions between healthcare settings.
Job Summary:The Per Diem Nurse Case Manager will be responsible for facilitating the coordination of care, from admission through discharge, with a focus on realistic treatment outcomes within the scope of available resources. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.
Key Responsibilities:- Assess patient health and psychosocial needs, conducting thorough evaluations per established guidelines.
- Develop and implement case management plans collaboratively with the interdisciplinary team, patient, and family to maximize healthcare responses, quality, and cost-effective outcomes.
- Communicate and coordinate effectively between team members, payers, and community resources to minimize fragmentation of services and promote optimal outcomes.
- Conduct patient-centered team meetings to coordinate healthcare team members and third-party payers, ensuring timely and informed decisions.
- Provide education to the interdisciplinary team, patient, and family about treatment options, community resources, insurance benefits, and psychosocial concerns.
- Empower patients and families to problem-solve and achieve desired outcomes, promoting patient self-determination.
- Ensure appropriate use of healthcare services and allocation of resources to improve quality of care and maintain a balance with cost-effective care on a case-by-case basis.
- Identify and provide access to necessary resources for discharge services, promoting a safe transition to the next most appropriate level of care.
- Develop discharge plans in collaboration with the healthcare team and family, meeting patient needs and financial requirements.
- Provide culturally competent care, incorporating patient cultural needs into the development of the discharge plan.
- Document all activities and outcomes in adherence to department standards.
- Adhere to third-party coverage guidelines for each patient situation.
- 5 years of related experience in case management, utilization review, or discharge planning in an acute, rehabilitation, skilled nursing, or outpatient setting.
- Current, unrestricted license to practice in Massachusetts in a health or human services discipline that allows the professional to conduct an assessment independently, as permitted within the scope of practice of the discipline.
- Knowledge of community services and healthcare systems, as well as criteria and levels of care.
- Computer proficiency, with Microsoft Office applications preferred.
- Certification in Case Management is preferred.
