">The successful candidate will be responsible for providing PT services, including assessment, treatment planning, implementation, and evaluation for our patients enrolled in our home health program in Santa Fe, NM, and surrounding areas. Key responsibilities include:
">- Evaluating patients' physical and functional abilities to develop individualized care plans">
- Planning, implementing, and modifying physical therapy treatment programs based on patients' medical referrals and assessments">
- Providing hands-on therapy, exercises, and techniques to help patients improve mobility, relieve pain, and restore function">
- Educating patients and their families on therapeutic exercises, proper body mechanics, and injury prevention techniques">
- Collaborating with interdisciplinary healthcare teams to ensure coordinated and holistic patient care">
- Maintaining accurate and timely documentation of patient evaluations, progress notes, and treatment plans">
Requirements
">To be considered for this role, you should have:
">- A graduate degree from an accredited School of Physical Therapy">
- A current State license as a Physical Therapist or a multi-state license issued by a state member of the National Physical Therapy Compact (PT Compact), if practicing in a state member of the PT Compact">
- A minimum of two years of experience as a Physical Therapist in a clinical care setting, with experience in Home Health, geriatrics, or medical-surgical preferred">
- Excellent interpersonal and communication skills">
- Strong nursing skills as defined by generally accepted standards of practice">
- Reliable transportation with valid and current auto liability insurance">
- Ability to work flexible hours and travel locally as needed">
- Ability to lift up to 50 lbs. and perform physical tasks associated with patient care">
