Job SummaryWe are looking for a dedicated Physical Therapist to provide high-quality patient care in a home health setting. As a Physical Therapist with LHC Group, you will have the opportunity to work with a diverse patient population and make a meaningful difference in their lives.
Responsibilities- Evaluate patient functional status and physical therapy needs, assisting the physician in developing a therapy plan of care.
- Develop and implement treatment plans to improve patient mobility, endurance, and overall well-being.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Monitor patient progress and adjust treatment plans as needed to achieve optimal outcomes.
- Current Physical Therapy licensure in the state of practice.
- Current CPR certification.
