About the Role

SaVida Health is seeking a skilled Care Coordinator to join our team in Bangor, ME. As a Care Coordinator, you will play a critical role in promoting the physical, social, and emotional health of our patients in an outpatient treatment setting.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments to identify patient needs and develop individualized treatment plans.
  • Collaborate with multidisciplinary teams to ensure seamless care coordination and effective communication.
  • Establish relationships with community agencies, hospitals, and service providers to link patients with necessary services.
  • Provide continuity of care through documentation and communication with collaborative providers and agencies.
  • Act as a patient advocate to resolve emergency problems and coordinate required services.
Qualifications and Requirements
  • Bachelor's Degree in counseling, social work, or a related field.
  • One year of experience working in the substance abuse treatment field.
  • One year of experience working in the mental health counseling field.
  • MHRTc license required.
Preferred Skills
  • Ability to maintain professionalism and work effectively in a team environment.
  • Strong communication and interpersonal skills.
  • Ability to plan and prioritize work with limited supervision.
  • Working knowledge of social services, justice, health, community service, and governmental agencies.
