Key Responsibilities- Conduct comprehensive psychosocial assessments to identify patient needs and develop individualized treatment plans.
- Collaborate with multidisciplinary teams to ensure seamless care coordination and effective communication.
- Establish relationships with community agencies, hospitals, and service providers to link patients with necessary services.
- Provide continuity of care through documentation and communication with collaborative providers and agencies.
- Act as a patient advocate to resolve emergency problems and coordinate required services.
- Bachelor's Degree in counseling, social work, or a related field.
- One year of experience working in the substance abuse treatment field.
- One year of experience working in the mental health counseling field.
- MHRTc license required.
- Ability to maintain professionalism and work effectively in a team environment.
- Strong communication and interpersonal skills.
- Ability to plan and prioritize work with limited supervision.
- Working knowledge of social services, justice, health, community service, and governmental agencies.
