Responsibilities- Evaluate physician medication orders for appropriateness, making recommendations for therapy changes as needed.
- Maintain accurate and up-to-date patient/resident databases, including current and past medical history, clinical and laboratory data, and concurrent medication.
- Collaborate with healthcare teams to provide clinical and technical support, ensuring safe and effective medication use.
- Implement policies and procedures for the safe procurement, storage, distribution, and use of drugs and biologicals.
- Document all communication with physicians, nurses, and other healthcare team members in the medical record.
- Bachelor's Degree in Pharmacy; Pharm D preferred.
- One year experience in long-term care.
- Licenses/Certifications: As required by government regulations and business needs.
