Job Responsibilities:

The successful candidate will utilize the nursing process to assess, plan, implement, and evaluate patient care in a Long Term Care/Assisted Living Facility. Key duties include:

  • Performing focused interviews to identify specific patient needs
  • Assessing signs and symptoms indicating physiologic and psychological changes in the patient's condition
  • Collecting, analyzing, and interpreting data and information from healthcare members and documenting actual and/or potential nursing diagnoses
  • Developing and implementing individualized patient plans of care
  • Providing a safe, comfortable, and therapeutic patient environment
