Responsibilities

  • Deliver high-quality physical therapy services to patients in their homes, promoting independence and improving functional abilities.
  • Conduct thorough assessments and develop personalized treatment plans to meet the unique needs of each patient.
  • Collaborate with our interdisciplinary team to ensure seamless care coordination and communication.
  • Provide education and support to patients and their families, empowering them to take an active role in their care.
  • Stay up-to-date with the latest clinical practices and guidelines, ensuring the highest level of care and quality.

Requirements

  • Current PT license as required by state.
  • CPR certification with American Heart Association or American Red Cross.
  • 1 year of experience in a healthcare facility.
  • Ability to meet the physical demands of the job.
  • Positive attitude and dedication to quality patient care.
  • Strong communication skills and ability to work effectively in a team environment.
  • Reliable transportation to perform job duties.
