Job Title: Nurse/LPN

AdaptHealth is seeking a skilled and compassionate Nurse/LPN to join our team. As a key member of our healthcare team, you will provide specialized nursing services to patients and caregivers in the comfort of their own homes or alternative settings.

Job Summary:

  • Provide technical nursing and skilled interventions as ordered by the physician.
  • Instruct, motivate, and assist patients/caregivers in learning and improving their ability to live independently at home.
  • Ensure physician orders are followed and included in patients' plans of care.
  • Continually monitor patient outcomes and report changes to supervisors to prevent hospital readmission.

Responsibilities:

  • Develop and revise plans of care based on physician orders.
  • Ensure nursing orders are carried out in a professional manner, satisfying professional ethics, patient rights, and State and Federal codes and regulations.
  • Participate in effective communication amongst other clinicians providing care to the patient.
  • Complete and submit all required documentation within established agency guidelines.
  • Cover after-hours, weekend, and holiday on-call to meet the branch's needs.

Requirements:

  • Associate degree in nursing or equivalent.
  • Current and unrestricted LPN clinical license.
  • Two (2) years of clinical nursing experience is required.
  • Five (5) years of related HME experience is preferred.
  • Valid and unrestricted driver's license.
