This role is essential for overseeing the formulation and execution of the Resident Assessment Instrument through the completion of the Minimum Data Set (MDS) in alignment with state and federal regulations, as well as the policies and objectives of the facility. This individual plays a crucial role within the interdisciplinary team, contributing to the assurance of quality care, continuity of services, and tailored patient-centered plans and objectives.
  • Performs comprehensive patient evaluations to ascertain health status, care level, and any necessary adjustments
  • Guarantees precise and prompt completion of the RAI
  • Facilitates interdisciplinary collaboration in the execution of the RAI and resident care strategy
  • Aids in the creation and implementation of initiatives and protocols aimed at optimizing the facility's reimbursement potential and enhancing care quality

Qualifications

  • Active RN licensure in NJ
  • Experience as an MDS Coordinator
  • Proficient in MDS Completion
  • Familiarity with Medicare and Medicaid regulations and benefit guidelines
