Key Responsibilities:
- Evaluate medication orders to confirm appropriateness based on patient-specific factors such as diagnosis, weight, and concurrent medications.
- Prepare and dispense medications according to established protocols and legal regulations.
- Identify and mitigate potential adverse drug reactions and interactions.
- Provide clinical recommendations to prescribers to enhance patient safety and therapeutic outcomes.
- Maintain comprehensive patient medication records and develop individualized care plans.
- Educate patients on medication use, including purpose, duration, and potential side effects.
- Serve as a resource for drug information within the healthcare team.
- Administer immunizations and participate in public health initiatives.
- Engage in quality assurance activities to uphold pharmacy standards.
- Oversee the work of pharmacy support staff and contribute to their training and development.
- Participate in ongoing professional education and training.
- Communicate effectively with healthcare professionals and patients to ensure optimal care.
- Maintain a clean and organized workspace, adhering to safety and regulatory standards.
- Ensure compliance with pharmacy operations and policies to prevent drug diversion.
Qualifications:
- Bachelor of Science in Pharmacy or Doctor of Pharmacy degree.
- Active pharmacy license in the state of Texas.
- Certification in immunization administration within six months of employment.
Preferred Qualifications:
- Three years of relevant pharmacy experience.
- Completion of PGY1 or PGY2 residency training.
Work Environment:
- Standard pharmacy setting with potential for weekend and shift work.
- May involve travel to various healthcare facilities.
- Exposure to occupational hazards typical in healthcare environments.
