Job Title: Physical Therapist - Home Health

LHC Group is seeking a skilled Physical Therapist to join our Home Health team in Fitchburg, Massachusetts.

Job Summary

We are looking for a compassionate and dedicated Physical Therapist to provide high-quality care to our patients in the comfort of their own homes. As a Physical Therapist with LHC Group, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Responsibilities
  • Evaluate patients' functional status and physical therapy needs, and develop individualized treatment plans to improve their mobility, endurance, and overall well-being.
  • Provide one-on-one therapy sessions to patients in their homes, using a variety of techniques and modalities to promote healing and recovery.
  • Collaborate with other healthcare professionals, including physicians, nurses, and social workers, to ensure comprehensive care and coordination of services.
  • Document patient progress and outcomes, and communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions.
Requirements
  • Current Physical Therapy licensure in the state of Massachusetts.
  • Current CPR certification.
  • Must have a current driver's license and vehicle insurance, and access to a dependable vehicle.
