Key Responsibilities:
• Provide clinical expertise in medication therapy management, including patient counseling and education on medication use, side effects, and interactions.
• Conduct thorough patient assessments to identify health risks and develop personalized treatment plans.
• Collaborate with healthcare providers to ensure seamless care transitions and optimize patient outcomes.
• Manage and maintain accurate patient records, including medication lists and treatment plans.
• Stay up-to-date on the latest pharmacy practices, laws, and regulations to ensure compliance and quality care.
• Participate in ongoing education and training to enhance clinical skills and stay current with industry developments.
Requirements:
• BS in Pharmacy or Pharmacist Degree from an accredited educational institution.
• Current pharmacist licensure in the states within the district.
• Experience performing prescription dispensing activities that demonstrate a strong working knowledge of applicable state and federal controlled substance laws.
• Certified Immunizer or willing to become an immunizer within 90 days of hire.
Preferred Qualifications:
• At least 1 year experience as a pharmacist in a retail setting, including prescription filling and verification, records and legal compliance, pharmacy operations, pharmacy software and technology systems, and insurance.
• Strong communication and interpersonal skills to effectively interact with patients, healthcare providers, and colleagues.
• Ability to work in a fast-paced environment and prioritize tasks to meet deadlines.
• Strong problem-solving and analytical skills to resolve complex patient care issues.
