Job Summary:

The successful candidate will be responsible for conducting assessments, developing and implementing individualized treatment plans, maintaining accurate documentation, and collaborating with other internal and external professionals. This role also involves participating in outreach and marketing efforts to promote services and attract new referrals.

Key Responsibilities:
  • Conduct initial assessments and administer required standardized screening tools.
  • Gather relevant information necessary to accurately diagnose and treat patients, including mental status exams, functional impairments, behavioral health, substance use, and trauma history.
  • Develop individualized treatment plans aimed at addressing identified mental health symptoms and presenting problems.
  • Employ culturally competent, trauma-informed best practices during psychotherapy.
  • Make essential referrals and connections to community resources through care coordination and by helping patients prioritize needs.
Requirements:
  • Master's Degree in Social Work, Counseling, Marriage and Family Therapy, or Psychology.
  • Active license in California (LCSW, LPCC, LMFT).
  • At least 2 years of therapy experience with crisis counseling and working with adults, children, and families in an outpatient setting.
  • Excellent written and oral communication skills, strong organizational, problem-solving, and analytical skills.
  • Ability to understand and adhere to established policies, procedures, protocols, and make timely and sound decisions.
