Job Summary

We are seeking a skilled Nurse Practitioner to join our Hospital at Home team. As a key member of our team, you will provide high-quality, evidence-based patient-centered care to a diverse patient population in a non-traditional setting.

Key Responsibilities
  • Provide inpatient-level acute care to patients in their homes, utilizing comprehensive assessments and physical exams to develop and implement individualized plans of care.
  • Collaborate with the Virtual Hospitalist to identify and assess potential Hospital at Home referrals, and conduct in-home patient visits to Hospital at Home patients.
  • Conduct review of potential patients via the electronic medical record and identify candidates for transfer to Hospital at Home, communicating with brick-and-mortar teams as needed.
  • Perform face-to-face evaluations and assessments of Hospital at Home admissions, completing admission notes and orders as required.
  • Describe and demystify the Hospital at Home model for eligible patients and caregivers, responding to questions and concerns in an empathic and reassuring manner.
  • Work closely with fulfillment to understand intake capacity in order to maximize daily program patient intake, quickly addressing obstacles to patient intake and participating in creative problem-solving and flexible care plan creation.
  • Act as an escalation point of contact for patient intake questions and troubleshooting, communicating and building collegial relationships with clinical staff and multidisciplinary colleagues.
  • Evaluate the referral and patient intake process to ensure the process is seamless, efficient, and effective for all stakeholders.
  • Collaborate seamlessly with and perform safe hand-offs to Virtual Hospitalists, Virtual RNs, and primary care physicians, demonstrating superior communication skills with patients and caregivers and practicing patient-centered care.
  • Promote professional practice and a culture of safety, reporting Sentinel Events and Serious Incidents, and participating in quality and safety reviews, root cause analyses, and other process improvement efforts.
  • Engage in continuous process improvement and program growth efforts, ensuring eligibility and assessment tools are accurate, up-to-date, and readily accessible.
  • Participate in ongoing learning and continuing education, utilizing advanced nursing knowledge to identify, prevent, and solve complex and/or recurring patient care problems.
Requirements
  • Master's Degree in Nursing.
  • Graduate of an NLN/AACN accredited program in nursing.
  • Minimum of five (5) years of experience with inpatient care or home-based setting.
  • Excellent written and verbal communication skills.
  • Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.
Preferred Qualifications
  • Institutional knowledge of the hospital or healthcare system is desirable.
  • Certifications in specialty fields such as acute care, ambulatory care, oncology, and/or geriatrics desirable.
License and Certifications Required
  • NJ State Professional Registered Nurse License.
  • Advance Practice Nurse License.
  • AHA Basic Health Care Life Support HCP Certification.
