Responsibilities

In this role, you will work closely with a physician to enhance the delivery of individualized primary healthcare services by:

  • Gathering patient history and conducting assessments through observation, interviews, and examinations.
  • Ordering, performing, and interpreting diagnostic tests.
  • Conducting preventative health assessments, screenings, immunizations, and patient care.
  • Providing direct treatment and management of health conditions, including referrals to other healthcare providers or community resources.
  • Educating patients and their families about preventative health, treatment options, and available community resources.
  • Documenting healthcare delivery and nursing processes in accordance with specialty standards and organizational policies.

Qualifications

Licensure/Certification/Registration:

  • BLS Provider certification required within 30 days of hire.
  • One of the following credentials is required:
    • Nurse Practitioner credential from the American Nurses Credentialing Center (ANCC), Indiana Board of Nursing, or National Certification Corporation, all within 180 days of hire.
    • Registered Nurse credential from the American Nurses Credentialing Center or Indiana Board of Nursing, obtained prior to hire.

Education:

  • A Master's degree in Nursing is required.
