Responsibilities
  • Conduct thorough patient evaluations in private residences, nursing facilities, and assisted living facilities
  • Provide inpatient and outpatient palliative care consults, collaborating with attending physicians and medical directors
  • Develop and implement individualized care plans that prioritize patient comfort and well-being
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
  • Participate in ongoing education and training to stay current on best practices in hospice and palliative care
Requirements
  • 2 years of experience as a Nurse Practitioner in an acute care environment
  • Masters of Science in Nursing - Family Nurse Practitioner
  • Active, unencumbered state NP license
  • Current driver's license and auto liability coverage
  • Interest in community-based care and willingness to travel between patient visits
  • DEA number and National Provider Identifier (NPI) required
