Key Responsibilities:
  • Assessment and Planning: Conduct thorough assessments of patients' needs, develop and implement individualized plans of care, and communicate effectively with patients, families, and healthcare providers.
  • Care Coordination: Collaborate with the interdisciplinary team to coordinate care, ensure seamless transitions, and promote patient safety and well-being.
  • Patient Education: Provide education and support to patients and families on disease management, self-care, and health promotion, empowering them to take an active role in their care.
  • Case Management: Identify and address patient needs, develop and implement care plans, and coordinate services to ensure optimal patient outcomes.
  • Documentation and Communication: Maintain accurate and timely documentation, communicate effectively with patients, families, and healthcare providers, and participate in interdisciplinary team meetings.
Requirements:
  • Education: Associate's degree in Nursing from an accredited program.
  • Experience: Minimum two years of experience as a Registered Nurse in medical-surgical, home health, or hospice settings.
  • Licensure: Registered Nurse License (Washington) required at hire or Compact License: Registered Nurse required at hire.
  • Certifications: Basic Life Support required at hire.
Preferred Qualifications:
  • Homecare System Experience: Familiarity with Horizon for homecare system.
  • Bachelor's Degree: Bachelor's degree in Nursing or related healthcare field.
