Job Summary
The RN-Continuum of Care Manager Peds is a professional nurse who has clinical experience with medically complex children and families. The nurse manages targeted pediatric populations to achieve efficient and effective care delivery through adherence to Case Management standards as outlined by the Case Management Society of America.
The nurse is accountable and responsible for direct and indirect patient care across the care continuum, including inpatient, ambulatory, post-acute and patient home settings. The nurse ensures continuity of care through defined, evidence-based methods, including but not limited to medication reconciliation, self-management plans, engagement and education of the patient and/or caregivers, referrals and other interventions.
Key Responsibilities
1. Facilitate pediatric patient-centered care by utilizing the nursing process to coordinate the care of an identified population of pediatric patients throughout the healthcare continuum.
2. Perform complete assessments of patient's current health status, including barriers and gaps in care.
3. Develop and implement plans of care and self-management plans to meet the needs of the patient and improve clinical outcomes and utilization patterns.
4. Coordinate patient and family participation in the development and implementation of the plans.
5. Provide education to patients and families to meet goals.
Interdisciplinary Practice
1. Function as part of the interdisciplinary team across the continuum, including acute, post-acute, and home settings. Regularly meet with the team to develop a comprehensive plan of care based on the needs of the patient and family.
2. Function as a resource to the community in areas of care coordination and utilization expertise.
3. Act as liaison for families and external agencies and individuals providing services to specific families. Facilitate referrals to other disciplines and internal and community-based programs as appropriate to improve outcomes.
Evidence-Based Care
1. Utilize and incorporate knowledge of efficiency and effectiveness indicators, such as PQRS, NCQA, URAC, and HEDIS, when coordinating and facilitating a plan of care.
2. Increase knowledge of best practices and clinical standards of care and incorporate knowledge into practice.
Measurements and Reporting
1. Document in the medical record and on team tools, accurately reflecting collaborative care planning, interventions, and evaluation against defined targets and goals.
Supervisory/Management Responsibilities
This is a non-management job that will report to a supervisor, manager, director, or executive.
Requirements
Bachelor's degree in Nursing
3 years of total nursing experience, with at least 1 year of Care Management/Case Coordination and 1 year of pediatric nursing experience preferred
South Carolina recognized RN License
SOUTH CAROLINA DRIVERS LICENSE
In lieu of the Above Minimum Requirements
In lieu of the BSN requirement, an Associate's Degree in Nursing may be accepted if the applicant agrees to enroll in an accredited BSN or MSN program within one year of their job effective date and obtain a BSN or MSN degree within four years of their job effective date.

Proof of current auto liability coverage
Must be willing to travel and have reliable personal transportation
Obtain case management certification (CCM) within 2 years of hire or eligibility to sit for the certification exam
