Job SummaryWe're seeking a Certified Nursing Assistant (CNA) to join our team. As a CNA, you'll play a vital role in providing personal care and support to our patients. You'll work closely with our RN Case Manager to ensure our patients receive the highest level of care.
Responsibilities- Perform personal care activities related to daily living, such as bathing, dressing, and grooming.
- Assist with lifting and transferring patients, as well as range of motion, positioning, exercise, and ambulation.
- Maintain electronic medical records and review patient care plans.
- Work under the direction of a Case Manager RN to ensure our patients receive the best possible care.
- Certified Nursing Assistant (CNA) license.
- 12 months of caregiving experience, preferably in a hospice or palliative care setting.
- Current CPR certification and a valid driver's license.
- Excellent customer service skills, strong communication, and problem-solving skills.
