We are seeking a skilled Cardiac Navigator RN to join our team at MercyOne. As a key member of our healthcare team, you will play a vital role in coordinating and providing care for high-risk individuals and populations in the inpatient and community settings.
Key Responsibilities- Evaluate patient referrals to develop individualized plans of care across the continuum of care.
- Act as a liaison among patients, families, community services, providers, and healthcare team members.
- Provide clinical leadership by precepting visiting professionals, students, and new graduates.
- Ensure collection and input of risk stratification data, reporting results to the healthcare team and participating in interpretation and formulation of action plans.
- Coordinate the healthcare team in development of clinical tools for optimal outcomes and report findings.
- Current Iowa registered nurse (RN) license.
- Demonstrated competency within a specialized area of clinical practice with 4 years' experience in that specialty.
- Proof of completion of Mandatory Reporter – Child and Dependent Adult Abuse training within three (3) months of hire.
- Basic Life Support certified within six (6) weeks of hire.
- Demonstrated success in positions of leadership.
