About the Role

We are seeking a skilled and compassionate Registered Nurse to join our team at Cornerstone Health Services, Inc. as a Hospice House Nurse. Key Responsibilities
  • Assess and identify patient and family needs, utilizing the nursing process to develop and implement individualized care plans.
  • Coordinate the Plan of Care with the Interdisciplinary Group (IDG) to ensure comprehensive and effective care.
  • Provide palliative and supportive care to patients and their families, incorporating psychosocial, spiritual, cultural, physical, and biological components.
  • Educate patients, families, caregivers, and other healthcare professionals about disease processes, palliative interventions, care giving, and safety practices.
  • Participate in the evaluation of hospice referrals and admissions to ensure level of care appropriateness.
  • Attend patient deaths, prepare bodies for transport, and oversee disposal of medications.
  • Provide bereavement interventions to families and caregivers in conjunction with psychosocial staff.
  • Communicate effectively with physicians, staff members, patients, families, and supervisors, utilizing positive approaches when working with others.
  • Supervise patient care given by other nursing team members, including LPNs, Hospice Aides, and contract nurses.
Requirements
  • Current Florida license as a Registered Nurse (RN), or current active single-state out-of-state nurse license, or current active multistate/compact nurse license.
  • Minimum one (1) year in-patient nursing or hospice experience for external candidates; for internal applicants, minimum of 6-months of LPN experience with hospice/home health within an affiliate.
  • Previous experience working with an Electronic Medical/Health Record (EMR/EHR) system.
  • Ability to use equipment with visual and auditory mechanisms.
  • Ability to effectively communicate in English (verbal and written).
  • Flexible schedule to meet the needs of Hospice House patients.
  • Ability to manage the emotional stress of working with and caring for terminally ill patients and their families.
  • Ability to physically access patient rooms and other areas of the Hospice House.
  • Ability to perform the essential functions and physical requirements of the job with or without reasonable accommodation.
  • Active Basic Life Support (BLS) for healthcare professionals from the American Heart Association or Red Cross.
  • Intermittent Driver - Valid driver's license and automobile insurance per Company policy.
