Job Title: Rehabilitation Therapy Assistant

Shift: Monday to Friday, Day Shift

Guaranteed Hours: 32 hours per week

Experience: New graduates are welcome; SNF experience preferred

License: State License Verification required

Certifications: Certified Occupational Therapy Assistant (COTA)

Must-Have:

  • COVID test 72 hours prior to start date, with results issued 48 hours prior
  • 8 hours of Dementia Training

Job Description:

  • Ensure the prescreen is filled out and attached during submission
  • Attach resume and license verification; candidates will be declined without these
  • Provide references (3)
  • Complete background checks, including:
  • County Criminal (7 years for all counties lived and worked)
  • Federal Criminal (7 years or more)
  • Multi-State Criminal Search (7 years or more)
  • Nationwide Sex Offender Search (7 years or more)
  • HHS/OIG Search
  • GSA Search
  • Social Security Number Trace
  • Provide documentation for health requirements, including:
  • 1 Step PPD or Quantiferon, or proof of Positive PPD and Chest X-Ray
  • Complete additional searches as required:
  • SAM Search
  • CORI Massachusetts Background
