About the Role

This part-time LPN position is responsible for providing high-quality patient care in our Rehabilitation department. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.

Responsibilities
  • Provide direct patient care, including administering medications, performing treatments, and monitoring patient progress.
  • Collaborate with the healthcare team to develop and implement individualized care plans.
  • Conduct thorough assessments of patients to identify their needs and develop effective care plans.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements

To be successful in this role, you will need:

  • A current RN license in the state of hire.
  • 1 year of experience as an LPN in a Rehabilitation setting.
  • Basic Life Support certification through the American Heart Association.
  • Ability to work weekends, with flexibility to work occasional holidays and overtime as needed.
