Job Summary:

You will work closely with patients, families, and interprofessional teams to develop and implement individualized care plans that address the unique needs of each patient.

Responsibilities:
  • Conduct comprehensive patient assessments, identify health risks, and develop evidence-based interventions
  • Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care and promote seamless transitions
  • Provide education and support to patients and their families on disease management, wellness, and prevention strategies
  • Participate in quality improvement initiatives, contribute to policy development, and engage in ongoing professional development to enhance practice
Requirements:
  • A bachelor's degree or higher from an accredited nursing program
  • One year of experience in a healthcare setting, preferably in a similar role or specialty
  • Current licensure as a Registered Nurse in Texas or Compact State; Temporary Licensure within 60 days if needed
  • Basic Life Support (BLS) certification; additional credentials may be required depending on unit/service line

Compensation and Benefits:

We offer a competitive salary range of $80,000 - $110,000 per annum, based on experience and qualifications.
