Key Responsibilities- Case Management: Develop and implement individualized care plans to meet the unique needs of patients.
- Patient Advocacy: Act as a liaison between patients, families, and healthcare providers to ensure effective communication and coordination of care.
- Clinical Expertise: Utilize clinical knowledge and experience to assess patient needs and develop strategies for optimal care.
- Collaboration: Work closely with interdisciplinary teams to ensure comprehensive and coordinated care.
- Documentation: Maintain accurate and timely documentation of patient care and outcomes.
- Education: Bachelor of Science in Nursing (BSN) or related degree required.
- Experience: Minimum 5 years of clinical experience in emergency department, critical care, or medical-surgical settings.
- Certifications: Registered Nurse (RN) licensure in New York State required; PRI/Screen certification and Certified Case Manager (CCM) certification preferred.
- Skills: Strong clinical knowledge, excellent communication and interpersonal skills, and proficiency in computer systems.
- Competitive Salary: $119,970 - $153,900 per year.
- Sign-On Bonus: $5,000.
- Full-time Benefits: Comprehensive benefits package, including health insurance, dental insurance, and vision insurance.
- Relocation Assistance: Available for eligible candidates.
