About the Role

We are seeking a skilled Physical Therapist to join our Home Health team at DCH Health System. As a Physical Therapist, you will be responsible for evaluating, planning, and implementing care plans for patients in their homes.

Key Responsibilities
  • Evaluate patient needs and develop individualized care plans
  • Collaborate with patients, families, and other healthcare providers to ensure comprehensive care
  • Perform therapeutic procedures and provide patient education
  • Monitor patient progress and adjust care plans as needed
  • Document patient information and maintain accurate records
Requirements
  • Bachelor's or Master's degree in Physical Therapy
  • Current licensure as a Physical Therapist in the state of Alabama
  • Basic Life Support (BLS) certification
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
Working Conditions

This position requires prolonged standing and walking, as well as the ability to lift and move patients up to 200 pounds. The Physical Therapist must also be able to work in a variety of environments, including patient homes, and be able to communicate effectively with patients, families, and other healthcare providers.
