Key Responsibilities
  • Provide direct patient care, including administering medications, wound care, and other treatments as needed.
  • Collaborate with interdisciplinary teams, including physicians, therapists, and other healthcare professionals, to develop and implement comprehensive care plans.
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing strategies for rehabilitation.
  • Communicate effectively with patients, families, and caregivers to ensure seamless care transitions and address any concerns or questions.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance patient care and outcomes.
Requirements
  • Current licensure as a Registered Nurse in the state of Minnesota.
  • Associate's or Bachelor's degree in Nursing.
  • Basic Life Support (BLS) certification.
  • Certified Rehabilitation Registered Nurse (CRRN) or equivalent certification within 5 years.
