BAYADA Home Health Care is seeking an experienced Physical Therapist to perform home health visits in Concord, NH. This role requires one year of prior clinical experience as a licensed Physical Therapist.

As a home care Physical Therapist, you will be part of a multi-disciplinary healthcare team providing rehabilitative care and skilled nursing to clients. BAYADA believes our employees are our greatest asset and invests in their growth and development.

Responsibilities:

  • Make home visits to clients in designated territories.
  • Perform diagnostic tests and measurements, such as mobility, range of joints, transfer status, and balance testing.
  • Implement integumentary protective interventions to prevent wound formation and promote wound repair.
  • Develop and implement individualized care plans, including manual therapeutic exercises and gait training.
  • Continually assess and revise the Physical Therapy care plan and participate in multidisciplinary care planning.
  • Educate and instruct clients and family members on rehabilitative care and activities.
  • Accurately document observations, interventions, and evaluations.

Qualifications:

  • A current New Hampshire Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills and prior experience with electronic medical records (EMR) preferred.
