LPN Homecare Specialist

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team in providing high-quality care to pediatric and adult patients in their own homes.
About the RoleThis is a dayshift position, working with patients who require skilled nursing care. As an LPN, you will be responsible for assessing patient needs, developing and implementing individualized care plans, and providing direct patient care.
Responsibilities- Assess patient needs and develop individualized care plans
- Provide direct patient care, including administering medications and treatments
- Monitor patient condition and report changes to physicians or supervisors
- Collaborate with healthcare team members to ensure comprehensive care
- Current LPN license for the state in which you practice
- Current Health Certificate
- Current PPD or Chest X-Ray
- Current BLS card
- One year prior LPN experience preferred
