Job Summary

This position offers a unique opportunity to support our inpatient units and grow your professional career within multiple specialty areas. As a Registered Nurse in our Weekend Program, you will be part of a dynamic float pool team at Grafton, providing exceptional patient care and contributing to a collaborative and supportive work environment.

Key Responsibilities
  • Utilize the nursing process to assess patient needs, plan and implement individualized interventions, and evaluate the effectiveness of the plan of care.
  • Include patients and families in planning and implementing care, developing and maintaining a therapeutic relationship throughout the healthcare continuum.
  • Anticipate and recognize significant changes in patient condition, status, and need for care, taking appropriate actions and collaborating with other healthcare team members to coordinate patient-centered care.
  • Implement strategies to reduce patient risk and increase patient safety, assessing patient and family readiness to learn and individualizing the approach as necessary.
  • Actively coordinate the discharge plan utilizing an interdisciplinary approach, applying evidence-based practice to deliver patient care and consistently incorporating nursing and regulatory standards of care into practice.
  • Provide ongoing evaluation of patient, team, and unit outcomes, empowered to take appropriate action when changes are needed, and supporting shared governance activities and initiatives to improve processes and patient outcomes.
  • Demonstrate effective communication, feedback, and conflict resolution skills, promoting collaboration among healthcare team members and assuming a leadership role in the care team, delegating appropriately to unlicensed personnel.
  • Promote a culture of safety through identifying threats to patient safety and intervening to prevent patient harm, reporting patient safety events and near misses in a timely manner, and seeking to identify potential safety issues and assisting in the implementation of corrective actions.
  • Apply ethical decision-making, demonstrating respect and understanding for peers and other clinical disciplines, and participating as an effective member of the patient care team to formulate an integrated, unbiased, individualized approach to care.
  • Participate in quality/process improvement initiatives to improve the functions of the department and the patient care experience, and participate in professional activities contributing to personal professional development and the development of others.
Requirements
  • Registered Nurse license issued by the state in which the team member practices, and Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA).
  • Completion of an accredited or approved program in Nursing.
  • Typically requires 1 year of experience in clinical nursing.
