Job Summary

The Triage Nurse Specialist reports to the Associate Clinical Director and is responsible for triaging patient and caregiver telephone calls, assisting in the solution, palliation of symptoms, and provision of education. This role requires strong time management and organizational skills, as well as excellent interpersonal and communication skills to interact with a diverse population.

Key Responsibilities
  • Triage all patient and caregiver calls, assisting in the solutions, palliation of symptoms, and provision of education.
  • Serve as a liaison with physician offices on behalf of the care team.
  • Provide immediate response and emotional support to the family at the time of death call.
  • Assist in the coordination of changes in patient level of care.
  • Communicate with insurance case managers on behalf of the care team, documenting discussions in detail.
  • Assist with pharmacy issues, ordering medication refills, securing scripts for controlled substances, and problem-solving breakdowns in systems communications.
  • Order supplies and durable medical equipment as appropriate and document same in the EMR.
  • Coordinate equipment pick up with vendor representatives as necessary.
  • Trend calls prospectively to determine patterns by team/provider and identify opportunities for improvement.
  • Deploy staff as needed to address patient and family-assessed needs.
Requirements
  • Diploma, ADN, and/or BSN from an accredited institution.
  • Current Maryland State license as a Registered Nurse.
  • One to two years of nursing experience, with Home Health Hospice or triage experience required.
  • Strong time management and organizational skills.
  • Excellent interpersonal and communication skills.
  • Ability to be patient and family-focused.
  • Knowledgeable of universal precautions for infection control and practices safe nursing standards for self-protection.
  • Ability to perform consistently in stressful situations.
  • Strong and creative problem-solving skills with the ability to prioritize and delegate.
  • Ability to simultaneously handle multiple assignments and projects with attention to details, efficiency, and accuracy and deadlines.

Pay Range: $30.48 - $46.85. Final salary offer will be based on the candidate's qualifications, education, experience, and alignment with our organizational needs.
