Job Summary

This is a grant-funded position for 15 years at Barton Health. The Licensed Clinical Social Worker (LCSW) will identify, triage, and manage patients with medical and behavioral health conditions within Barton Community Health Center and Barton Psychiatry & Mental Health offices.

Key Responsibilities
  • Provide skill training through psycho-education strategies and brief interventions based on goals set in partnership with the patient.
  • Collaborate with the Patient Centered Medical Home (PCMH) to provide patient-centered behavioral health services.
  • Apply specialized clinical knowledge and advanced clinical skills in prevention, assessment, and treatment of mental, emotional, behavioral, and addictive disorders.
  • Engage in multidisciplinary treatment planning and clinical social work interventions.
  • Work with the substance use disorder care team.
Qualifications
  • Masters in Social Work from an accredited institute.
  • Minimum two years of experience as a clinical social worker preferred.
  • Ability to perform evidenced-based therapies with little to no supervision.
  • Knowledge of community resources.
  • Detailed knowledge and experience with evidenced-based group and individual therapies.
  • Working knowledge of psychotherapy billing and coding.
  • Ability to effectively communicate in English, in compliance with patient safety standards.
  • Bilingual abilities preferred.
Certifications/Licensure
  • Current licensure with the Board of Behavioral Sciences as an LCSW in the state of California, dual license in Nevada preferred, in good standing.
  • Current CPR/BLS certification by the American Heart Association (BLS for the Healthcare Provider) or ability to obtain prior to start date.
